Discharge Planner

Join our team as an Outpatient Discharge Planner! This role is responsible for planning and coordinating activities related to discharge planning, developing and implementing a plan of care designed to meet the patient’s post-hospitalization needs. The Discharge Planner will communicate with referral sources, outside professionals, and families to ensure continuity of care.

Requirements

• Bachelor's Degree from an accredited college or university in Social Work or a clinical-related mental health field
• Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred
• CPI or Safety Care training within 90 days of employment and before assisting in a restraining procedure
• CPR/First Aid (BLS) certification at the time of application or arrangements can be made to attend the course on campus
