Case Manager (RN)

The Case Manager leads care coordination throughout hospital levels of care, completing assessments and transition care planning for patients and their families. The position also assists physicians and finance with resource utilization and management.

Requirements

• Licensed RN in the State of California
• At least 2 years clinical patient care experience
• Certification in Case Management or Utilization Review
• Good written and verbal communication skills
• Ability to manage priorities and workflow
• Proficiency with computers and electronic forms of communication
