RN Case Manager- Full-time, Day Shifts (Temple University Hospital - Jeanes Campus)

Collaborate with patient/family, physicians, and interdisciplinary team to facilitate efficient and cost-effective patient progress through acute episode of care. Monitor level of care, initiate evidence-based protocols, and propose transition plan within 48 hours of admission.

Requirements

• Graduate of accredited school of nursing
• Bachelor's Degree BSN
• 1 year experience as a Registered Nurse in an acute care setting
• 2 years experience in a Hospital setting as a Case Manager
