Provide social work services to patients and families in the acute care setting to meet their medically related social and emotional needs. Identify patients in need of social work intervention, assess patients, and develop intervention and discharge plans.
Requirements
- Identify patients in need of social work intervention within 24 to 48 hours of admission based on high risk screening criteria.
- Follow up on all referrals made to social work from all sources other than high risk screening within 24 hours of referral.
- Assess patients to determine the bio-psychosocial factors that impact their medical condition, treatment, recovery, and safe transition from one care environment to another.
- Develop intervention plan and discharge plan to meet patient’s/family’s needs.
- Provide social work services to patients in OB and takes weekend on call for OB/emergency department.
