This role focuses on improving patient outcomes through consistent follow-up calls, coordinated visits, and care plan management. Requirements

• Participate in rotation of extended access hours including late nights, weekends and holidays
• Complete and stay up to date on yearly competencies for hands on skills
• Complete all required and requested patient forms as needed
• Participate in continuing education activities and conferences
