Job Description
Case Finder utilizes clinical expertise to assess, provide, and monitor healthcare services for patients. Ensures medical appropriateness criteria are met and coordinates patient discharge teaching. Acts as a liaison between patients, families, and healthcare providers.
Requirements
- Current California Registered Nurse (RN) Licensure or Licensed Vocational Nurse (LVN) licensure required
- Current basic cardiac life support (BCLS) certification
- Minimum of (1) years of acute or subacute nursing experience required
