RN Case Manager – Hospice Care

Transitions Hospice provides compassionate and unhurried hospice care, protecting dignity and comfort of patients and their families. RNs coordinate care, guide teams, and show up when it matters most.

Requirements

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication skills across care teams and families
