Requirements

• Bachelor's Degree in Pharmacy or PharmD
• Completion of ASHP-accredited residency program preferred
• 1 year of clinical patient care experience
• 3 years of institutional experience (hospital or integrated health system) or ASHP residency training program preferred
• Basic Life Support (BLS) certification
• Board Certification (preferred)
