Integrated Health Coach - Vanderburgh County

Role Overview

Vanna Health is seeking a Coach to support individuals with behavioral health challenges in Vanderburgh County. The Coach will engage with members, build trusting relationships, and provide coaching and advocacy to help them thrive. The role involves working with clinicians and specialists to ensure members have access to high-quality care.

What You Will Do

Responsibilities include engaging and building trust with members, achieving enrollment goals, supporting recovery and self-determination, and providing feedback for continuous improvement. The Coach will also deliver physical health coaching and encourage wellness strategies.

Why It Might Be a Fit

The ideal candidate has over two years of experience working with individuals with serious mental illness, experience in Vanderburgh County, and a valid driver's license. Spanish fluency and Certified Peer Specialist Training are nice to have.

Requirements

• Over two years of experience working directly with people with a serious mental illness diagnosis
• Experience working in and around the communities of Vanderburgh County
• Valid driver's license
• High school diploma or equivalent
