Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility.
Requirements
- Conduct comprehensive psychosocial assessments for residents upon admission and as needed
- Develop, implement, and monitor individualized treatment plans
- Provide individual, group, and/or family therapy sessions
- Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
- Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
- Collaborate with the Vitae clinical team, the patient’s attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care
- Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
- Provide crisis intervention and behavioral management strategies when needed
- Educate staff and families on mental health conditions and effective communication strategies
- Ensure compliance with HIPAA and ethical standards of practice
