The Grant Case Manager is responsible for tracking unsheltered guests' progress through case management and ensuring the effective administration of various grant programs.
Requirements
- 2-3 years' professional experience in case management
- Knowledge of community resources
- Strong professional and interpersonal communication skills
- Experience with county database management in a community setting
- Intermediate knowledge of office technology tools
- Demonstrated ability to organize and track data for program outcomes
- Fluency in Spanish (preferred)
