Case Manager

The Grant Case Manager is responsible for tracking unsheltered guests' progress through case management and ensuring the effective administration of various grant programs.

Requirements

• 2-3 years' professional experience in case management
• Knowledge of community resources
• Strong professional and interpersonal communication skills
• Experience with county database management in a community setting
• Intermediate knowledge of office technology tools
• Demonstrated ability to organize and track data for program outcomes
• Fluency in Spanish (preferred)
