Lead Care Manager, Enhanced Care Management

We are seeking a Lead Care Manager to provide direct service and coordinate care management for the Enhanced Care Management Program (ECM) at Wayfinder Family Services. The successful candidate will oversee specific cases, coordinate health care benefits, provide education, and facilitate client access to care in a timely and cost-effective manner.

Requirements

• Bachelor's degree from an accredited university in a related field
• At least three years of relevant work experience
• Multilingual capabilities are preferred but not required
• Prior experience as a community health worker, peer support specialist, medical assistant, or similar role is a plus
