
Job description
Performs initial screening and functional assessment for individuals referred to the Weld County Case Management Agency (CMA) for long-term services and supports. Develops Person-Centered Individualized Support Plans (PCISP) for eligible individuals and assists in determining appropriate programs for service. Provides ongoing case management services to residents of Weld County and their families to enhance quality of life and meet long-term care needs.
Completes initial screenings, develops PCISP, authorizes long-term home health services, maintains communication with service providers, assists members and families in locating and selecting service providers, and coordinates and facilitates services.
Ideal candidate is self-motivated, values collaboration, highly organized, and dependable. Must possess strong attention to detail and ability to learn and apply new computer skills. Bilingual preferred but not required.
