Blended Enhanced Targeted Case Manager

The Blended Enhanced Targeted Case Manager will assist clients in gaining access to needed resources such as medical, social, educational, and other supportive services. The position involves assessing consumer needs, providing advocacy, counseling, and linking clients with community resources.

Requirements

• Bachelors or Masters Degree in Human services or related field
• Registered Nurse
• High School Diploma and 12 semester credit hours in human services or related field
• Two years with one year in direct client contact
• High school Diploma and five years of mental health care experience
• Employment as an intensive case manager prior to April 1, 1989
