Family Hub Practitioner

We're looking for a Family Hub Practitioner to join our team in Wiltshire, UK. As a Family Hub Practitioner, you will provide one-to-one and group support to strengthen families and build resilience. You will work in partnership with schools, health professionals, and community services to get families the right help at the right time.

Requirements

• Hold a Level 3 or above qualification in social work, family studies, youth and community work, psychology, or a related field.
• Bring experience of supporting children, young people and families, including case management and tailored support planning.
• Have strong knowledge of safeguarding, child protection, and early intervention practices.
• Communicate with confidence and compassion, building trust with families and professionals alike.
• Organise and prioritise a busy caseload, keeping accurate records and delivering positive outcomes.
• Work collaboratively with schools, health services, and community partners to coordinate support.
• Be adaptable, empathetic, and resilient, with a genuine commitment to empowering families.
• Able to travel independently and reliably across the South Wiltshire area to meet the demands of the role.
