Requirements

• Utilize clinical skills to promote wellness and prevent re-hospitalization.
• Perform a full assessment of patient & family
• Develop advanced care plans and rehabilitation strategies.
• Collaborate with physicians and agency staff.
• Prepare clinical notes, orders, and charting.
• Manage patients' care episode and interdisciplinary team.
• Case manage patients care episode.
