Job Description
The Patient Navigator LPN will assist in the assessment, planning, implementation, evaluation, and documentation of nursing care in accordance with facility, department, and professional nursing practice.
Requirements
- Guides patients and families through their healthcare experience, answers questions, provides education and helps them understand their diagnosis and treatment options.
- Follows patients from date of diagnosis through the completion of the care-path to ensure all treatment options are explored.
- Reviews prior patient history and provides updated information in the medical record in an appropriate time frame.
- Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and participating in professional societies.
