Job Description
AIM is seeking a Service Coordinator to support its mission-driven organization in Elmira, NY. The role involves home visiting, community education, and case management for individuals with disabilities and the elderly. The position offers a full benefit package and growth potential for full-time.
Requirements
- Comply with all aspects and responsibilities of the NY State Department of Health (DOH) NHTD and TBI service coordination waiver services.
- Home visiting and community education.
- Develop, review, and submit service plans.
- Maintain orderly recordkeeping with strict attention to detail and articulate time tracking.
- Participate in the development of the programs.
- Case management for consumers participating in services. Up to 25 individuals.
