Physical Therapist
Bachelor's/Master's in PT. Valid AZ PT license. Multidisciplinary experience preferred
Interventional Pain Physician
Board certification or board eligibility in Pain Medicine (Anesthesiology, Physical Medicine, and Rehabilitation) with a Medical degree (MD or DO) from an accredited medical school
Medical Referral Coordinator
High school diploma, 2+ years of experience in healthcare, strong understanding of medical terminology
