Population Health Manager

The Population Health Manager will lead initiatives to improve and grow patient complexity capture, increase usage of category two codes, and oversee reporting for value-based care, among other responsibilities.

Requirements

• Bachelor's degree in nursing or Health Administration or related field
• Registered Nurse
• Two (2) years of relevant experience in population health-related programs
• MS in Health Administration or comparable experience
• Experience with Medicare, Medicaid, and/or commercial value-based payment models
• Regulatory and contractual requirement management experience
• Previous management/supervisory experience
