Case Manager, Safe Options Support (SOS), The Rockaways

Breaking Ground is a non-profit organization that provides supportive housing and services to people experiencing homelessness. The Case Manager, Safe Options Support, will provide case management services to individuals experiencing homelessness, some with mental illness and/or substance use disorders, to assist them in transitioning to permanent housing.

Requirements

• Provide direct services to a caseload of approximately 25 clients
• Conduct psychosocial evaluations
• Connect clients to psychiatric services and medical treatment facilities
• Obtain identification, public benefits, and other applicable income sources for clients
• Maintain record keeping in compliance with agency standards and City and State agencies
