Job Description
Care Dimensions is seeking a passionate and dedicated Registered Nurse to join their team as a Case Manager. This role focuses on providing compassionate care to patients with advanced or end-of-life illnesses, and involves coordinating patient care, developing care plans, and advocating for patients and families. Requirements
- Active Massachusetts RN license in good standing
- Associate’s degree in Nursing required; BSN or equivalent preferred
- Proven ability to provide care with compassion and cultural sensitivity
- Strong interpersonal skills and the ability to thrive in a collaborative team environment
- Valid driver’s license and access to an insured, reliable vehicle
