RN Case Manager

Care Dimensions is seeking a compassionate and dedicated Registered Nurse to join their team as an RN Case Manager. The RN Case Manager will work under the supervision of a Clinical Manager to coordinate patient care, develop comprehensive care plans, and advocate for patients and families.

Requirements

• Active Massachusetts RN license in good standing
• Associate’s degree in Nursing required; BSN or equivalent preferred
• Valid driver’s license and access to an insured, reliable vehicle
