RN Case Manager

Care Dimensions is seeking a compassionate and dedicated Registered Nurse to join their team as an RN Case Manager. The role involves coordinating patient care, conducting assessments, and developing comprehensive care plans. The RN Case Manager will work under the supervision of a Clinical Manager and be part of a collaborative, team-based work culture.

Requirements

• Active Massachusetts RN license in good standing
• Associate’s degree in Nursing required; BSN or equivalent preferred
• Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
• Strong interpersonal skills and the ability to thrive in a collaborative team environment
• Flexibility, self-direction, and sound clinical judgment
• Valid driver’s license and access to an insured, reliable vehicle
