Job Description
As a Hospice Community Care Coordinator, you'll help patients understand their care options and take the next step toward better health at home. You'll build lasting relationships with patients, especially in underserved communities, and report trends and insights to refine messaging and remove systemic barriers.
Requirements
- High school diploma or equivalent required
- Experience working with underserved or high-risk populations
- Strong communication and interpersonal skills
- Comfort working independently in the field with reliable transportation
- Familiarity with documenting in EMR or CRM platforms
