LICENSED PRACTICAL NURSE-Continuous Care - Home Health of Dade

The Licensed Practical Nurse (LPN) provides direct and indirect patient care in the home, under the direction of a Registered Nurse. This role involves applying standard clinical practices, utilizing holistic approaches to treatment, and ensuring high-quality care. The position involves working with patients and their families as part of a healthcare team, and adhering to facility policies and regulations.

• Evidence of ability to coordinate patient care, strong interpersonal skills, problem solving ability, decision making ability and ability to work closely with others.
• Proficient with use of computers
• Must be able & willing to work a flexible schedule
• Excellent oral and communication skills
• Must have knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to home health care
• Must be knowledgeable of computer office/clinical software
• Must be able to read, write and understand the English language
