Community Outreach Counselor

The community outreach specialist is responsible for developing and implementing outreach strategies that increase awareness of Centerstone services to community members. This role builds and maintains relationships with community members, attends events, and conducts comprehensive outreach services to include intervention, education, and linkages to services.

Requirements

• Knowledgeable on Centerstone programs and services, eligibility requirements, and availability of services
• Ensures continuity of care for clients by coordinating with team members, families, Centerstone colleagues, and community partners
• Increase contacts with community organizations to increase client referrals and maintain relationships
• Complete the enrollment and assessment process for eligible participants interested in Centerstone's programs
• Conduct ongoing assessment and evaluation of community needs and changes
• Implement strategies to inform the public about the mission of the organization
• Responsible for the development of outreach strategies that increase awareness of Centerstone services to community members
