Job Description
Provides person-centered assistance to clients in the development of life skills and personal goals. Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their families.
Requirements
- Coordinates client care by working closely with a multidisciplinary team
- Teaches and trains clients in activities of daily living
- Connects client with community support systems
- Provides telephone and on-site crisis intervention/prevention and follow-up
