Job Description
Conviva Primary Care is looking for a Nurse Practitioner to join their team-based care environment in Coconut Creek, FL. The successful candidate will work in a value-based care provider focused on quality of care for seniors. The Nurse Practitioner will work in a concierge experience, providing a multidisciplinary service and coordinated care supported by analytics and tools.
Requirements
- Nurse Practitioner with Board Certification
- Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
- Valid, unrestricted license in the state of desired employment (or willing to obtain)
- Current, unrestricted DEA license
