Community Reach Center is seeking a Case Manager to join their ACT Team. The Case Manager provides clinical and support services to consumers and their families through integrated behavioral healthcare services. The role emphasizes assisting individuals to develop social, occupational, leisure, and living skills.
Requirements
- Bachelor's degree in psychology, Social Work, or directly related field
- 6 months of experience working with adults who have mental illness, particularly those individuals diagnosed with a serious and persistent mental illness
- Communication, organization, time management, and clinical skills
- Ability to work independently in a community-based setting
- Valid Colorado Driver's license with a clean driving record
- Bilingual Spanish preferred
