Job Description
ConcertoCare is seeking an Advanced Practice Provider – Nurse Practitioner or Physician Assistant to deliver in-home and community-based care to patients with complex needs. Requirements
- Nurse Practitioner with graduation from an accredited health professions school and successful completion of ANCC or AANP certification; or board-certified Physician Assistant.
- Active licensure in applicable state(s).
- Active DEA licensure and state-specific controlled substance registration, as applicable.
- Average to advanced computer and software skills.
- Ability and means to travel.
