4+ years of post-residency clinical experience, with leadership and team management expertise, and board certification in Internal Medicine or Family Medicine

Experience in delivering in-home care, virtual care via telehealth, and managing complex medical needs

LPN or RN license, CPR and BLS certifications, and at least 1-5 years of experience

Advanced Practice Provider – Nurse Practitioner or Physician Assistant requires 2-5 years of experience and a Master's degree in Nursing or a related field

3+ years of experience in-home/virtual experience, geriatrics and/or palliative medicine training, and managed care and/or value-based healthcare delivery experience

Must have an ANCC/AANP certification or board certification as a PAs or a recent graduation from an accredited school. Must be licensed and have a DEA and controlled substance registration. Requires approx. 5+ years driving experience and access to public transportation

Must be a Licensed Practical Nurse. CPR/BLS certification required. Computer skills are a plus

NPA certification/LMNP, DEA license/state req, geriatrics/palliative medicine training preferred, BLS certification req. Computer skills are vital

2+ years of experience providing care to aging adults or individuals with complex medical needs, with proven experience in delivering in-home care and virtual care via telehealth
