Job Description
ConcertoCare is seeking a Bilingual Community Health Worker (LPN) to provide home and community-based care to patients with complex needs. The role involves delivering care through clinical services, coordination, and support to patients in their homes and fields, offering a collaborative team-based approach and fostering a holistic approach to care.
Requirements
- Licensed Practice Nurse (LPN)
- Unencumbered license certification in the state of employment
- Current CPR and BLS certification
- Experience providing care for the whole person, including patients living with mental illness and substance use disorders.
- Average to advanced computer & software skills (Microsoft Word, Excel, Outlook)
- Ability and means to travel as needed.
- Driver's license for a minimum of 5 years, with vehicle-related insurance and proof of liability/property damage.
