Job Description
ConcertoCare is seeking a Nurse Practitioner or Physician Assistant to be a core member of a multi-disciplinary field team. This role involves providing in-home and virtual clinical services to patients with complex care needs, emphasizing patient-centered care, collaborative work, and professional development. The position offers a blend of in-person and virtual care, supportive systems, and a focus on innovative value-based healthcare models.
Requirements
- Nurse Practitioner or Physician Assistant license in applicable state(s).
- Active DEA license and state-specific controlled substance registration.
- Advanced training in geriatrics and/or palliative medicine preferred.
- Certification in basic life support (BLS) required.
