Job Description
The ICMS Case Manager coordinates and manages the care of consumers in Integrated Case Management Services (ICMS). Through community outreach and home visits, the Case Manager provides ongoing support to consumers to help them function independently and access mental health services.
Requirements
- Master's degree in social work, counseling, psychology or related field
- 1 year of relevant supervisory experience
- Ability to manage multiple priorities and work in a fast-paced environment under tight deadlines
- Able to fully utilize technology, including Electronic Health Records
- Possess a valid driver's license
- Strong verbal and written communication
- Compassionate, flexible, and client-focused approach to care
