Referral Coordinator - Central NJ

Referral Coordinator plays a vital role in connecting Forge Health's mission to the community by building partnerships and driving awareness of our behavioral health services across Central NJ. As a collaborative member of both the clinical and growth teams, this individual develops outreach initiatives, strengthens relationships with providers and community organizations, and helps ensure individuals can easily access the care they need.

Requirements

• 2+ years prior experience in behavioral health, Healthcare outreach, provider relations, or business development.
• Working knowledge of behavioral healthcare models, referral processes and provider workflows.
• Valid state license and access to a vehicle.
• Excellent communication, presentation and relationship-building skills.
