Role Overview

HarmonyCares is a leading national value-based provider of in-home primary care services. We seek Nurse Practitioners with experience in home health, geriatrics, hospice, and primary care to provide patient-centered care in a team-based model.

What You Will Do

Provide primary care to high acuity patients in their home Monday - Friday, with minimal call responsibilities and an assistant to support appointments.

Requirements

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
