Geriatric Medicine Physician

We are seeking a Geriatric Medicine Physician to provide medical care services at skilled nursing facilities, rest homes, assisted livings, and home settings. The physician will work collaboratively with the Extended Care Community Program's nurse practitioner(s) and interdisciplinary teams.

Requirements

• Graduate degree from an accredited School of Medicine and Residency Program
• Board-certified/board-eligible in internal medicine or family medicine with a focus on geriatric medicine
• Valid medical license
• Ability to use medical record systems to thoroughly and properly document patient visits
