Nurse Care Manager

The Health Federation of Philadelphia is seeking an experienced Nurse Care Manager to join a new care management program aimed at reducing health inequities and improving health outcomes for low-income patients struggling with chronic physical and mental health conditions. The position will be part of a team of four and will work with community health centers and a Medicaid Managed Care Organization to achieve improved outcomes for health center patients.

Requirements

• Bachelor of Science in Nursing or Masters-level training
• Licensure as a Registered Nurse in Pennsylvania
• At least 2 years of experience as a nurse care manager
• Demonstrates customer focused interpersonal skills
• Demonstrates ability to work autonomously and be directly accountable for practice
• Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making
• Adept with technology and data, including electronic health records, payer portals, Microsoft office, and Google apps
