Case Manager

We are seeking a Case Manager to help veteran families remain housed or obtain permanent housing and support the household with ongoing services to ensure housing stability.

Requirements

• Conducting assessments and program intakes and preparing housing stability plans
• Identifying barriers and challenges and utilizing a solution-based approach
• Providing advocacy for public benefits and accessing financial assistance
• Maintaining and serving as liaison to a network of resources and contacts
• Conducting outreach to engage and enroll eligible Veteran families
