Primary Care Physician

We are seeking a Primary Care Physician to join our interdisciplinary care team. The ideal candidate will have a passion for building and innovating, with a focus on female-focused, age-conscious care. The Primary Care Physician will evaluate patients' acute and chronic conditions, manage medications, mental health, and socio-emotional needs, and provide empathetic patient education.

Requirements

• Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from the 50 United States or an accredited foreign medical school with proof of ECFMG certification
• Board certified in Internal Medicine or Family Medicine; Fellowship in Geriatrics desirable
• Current, active physician (MD/DO) license from the Minnesota Board of Medical Practice or willingness to obtain upon hire
• Active DEA registration with controlled substances prescriptive authority within the state of Minnesota or willingness to obtain upon hire
• Basic Life Support (BLS) certification or willingness to obtain
• Familiarity with population health and experience in achieving quality metrics
• Experience or knowledge in value-based care models desirable
