Job Description
Community Integration Professional I supports individuals in a Day Habilitation setting to achieve their goals and participate in community activities. This is an entry-level position that requires no prior experience and provides training.
Requirements
- Collaborate with individuals to create meaningful daily activities
- Identify and plan individualized daily and weekly schedules
- Participate in community activities related to hobbies, leisure, and personal health
- Provide services based on unique goals and behavioral care plans
- Communicate effectively with the team and individuals served
- Be an advocate for individuals' rights and independence
