The Home Health Physical Therapist provides high-quality, patient-centered physical therapy services to individuals in their homes, evaluating, diagnosing, and treating patients with physical impairments and mobility issues, and creating personalized therapy plans.

Requirements

• Valid Physical Therapy license in the state where work is performed.
• 1 or more years of physical therapy experience, preferably in a home health or similar setting.
• Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.
• Ability to lift and transfer patients, with or without assistance, up to 50lbs.
• Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.
• Ability to respond swiftly and efficiently to emergency situations.
• Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.
