The Hospice RN Case Manager coordinates and manages the care of patients in a hospice setting, providing skilled nursing services and overseeing the implementation of care plans.
Requirements
- Current Registered Nurse (RN) license in the state of practice
- Minimum of one year of experience as an RN in a hospice or related healthcare setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation for home visits
- Current CPR certification
- Knowledge of regulatory requirements and guidelines governing hospice care and documentation
- Demonstrated commitment to providing compassionate, patient-centered care in a hospice setting
- Experience with electronic medical record systems and documentation preferred
- Physical ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds
- Ability to respond swiftly and efficiently to emergency situations
- Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager
- Ability to use abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing
