Job Description
Monogram Health is seeking a skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver personalized medical care to patients with complex chronic conditions in the comfort of their own homes. The ideal candidate will have a passion for putting patients first and making a meaningful impact on their lives.
Requirements
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- 2 or more years of direct patient care experience
- Managed Care/IPA/Health Plan experience
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
