Social Worker - Discharge Planner

As a Social Worker, you will advocate for the well-being of our residents and ensure their holistic needs are met, and play a pivotal role in enhancing the quality of life of our residents.

Requirements

• Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, as well as strengths and preferences
• Collaborate with residents, their families, and the interdisciplinary team to develop individualized care plans that address social, emotional, and psychological concerns
• Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, adjustment to care transitions, and decision-making processes
