
Job description
Provides individual and group work services to patients and their families in various healthcare settings, utilizing psychosocial assessment skills to deliver social work services.
Deliver social work services, provide psycho education, counseling, and crisis intervention, formulate and implement treatment and discharge plans, and maintain professional patient care documentation.
Requires a Master of Social Work degree, current licensure to practice as a Master Social Worker or Clinical Social Worker in New York State, and 1-3 years of relevant experience.
