Job Description
This Social Worker/Discharge Planner assists in resource management for patients through collaboration with the multidisciplinary team, ensuring coordinated patient care and discharge planning. They will assess patients, provide psychosocial support, and arrange post-hospitalization services.
Requirements
- Master's degree in social work (MSW)
- One (1) year of social work or resource management experience in an acute healthcare and/or mental health setting
- Strong working knowledge of managed care and community resources and resource management
- Knowledge and expertise in working with patients with mental health concerns and psychosocial stressors
- Current licensure in the state of Mississippi
- BLS certification
