Resident Service Coordinator - 35 Hours Per Week - Up to $21/hour

The Resident Service Coordinator is a social service staff person who links elderly and/or disabled residents to supportive or medical services in the community. The position involves developing contacts with service providers, educating residents on available services, and monitoring provisions of services.

Requirements

• Assists and advises residents and families on necessary services
• Promotes wellness activities for all residents
• Educates residents, families, and staff on available community resources
• Acts as a liaison between community agencies, service providers, and residents
