Service Coordinator Clinician - Remote to Cass County, Indiana Travel Required

We are seeking a Service Coordinator Clinician to join our team in Marion county. In this role, you will conduct assessments, identify member needs, and develop person-centered service plans. This position requires 50% travel and 50% remote office work.

Requirements

• Current, unrestricted RN, LSW, LCSW, or LMHC license in applicable state(s) required.
• Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role;
• State-specified certification based on state law and/or contract may be required.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.
• Compassionate and patient-centered approach to care.
